• Arrive 10-15 minutes early to complete paperwork if you don’t have a chance to download it from our webpage. There are 5 documents that you will need to print, complete and sign.
    • Patients with Medicare insurance should arrive 20 min early as there may be additional paperwork to complete
  • Bring your insurance card (or claim number) and photo identification
  • Provide the receptionist with the prescription (referral slip) from your referring medical provider
  • Remember to wear or bring comfortable clothing
    • Shorts or loose sweatpants if you are being treated for back/hip or other lower extremity issue
    • Female patients are encouraged to wear or bring sports bra or tank if being treated for neck or shoulder issues.
    • Patient gowns are available for your convenience
  • Be prepared to spend 1 hour at the first appointment.  Your past and current medical history will be taken and a physical exam will be performed to determine the best course of treatment.
  • In most cases a home program will be assigned after the first visit.  This may include pain reduction techniques, stretching or exercises specific to your diagnosis or problem.
